Screenings:

A Program of Screening for Mental Health, Inc. and type the keyword Sample

Screenings include: Depression, Bipolar Disorder, Alcohol, Eating Disorders, Generalized Anxiety, and Post-Traumatic a Stress. While taking a questionnaire, you can opt to click on "No response" if a question is not applicable to you.
